redclyffecareplan

A personalised care plan is completed in consultation with the individual and with permission, those closest to them. This details all about the person, their interests, what's important to them and what's important for them. It also describes their goals and how we can help achieve them. The care plan is a live document which we develop in line with the individual's changing wants and needs.

We work with individuals to facilitate choice making; develop skills and gain independence. We support access to a range of community resources and facilitate leisure, education and work opportunities. Working in partnership with care managers, the local learning disability team, relevant medical professionals and family and support networks, we ensure holistic support and continuity of care.

Our staff team are trained in accordance with individuals' needs in addition to mandatory and statutory requirements. We provide care and support in keeping with regulatory standards and best practice guidance. All staff members undergo an intensive training programme overseen by our onsite Training Coordinator, Claire Roper.
